Services

The purpose of Aralife Case management Services is to enhance the quality of life for individuals with a severe and persistent mental illness and/or co-occurring disorder (mental health) by providing, promoting and evaluating a comprehensive array of fully integrated quality resources and services suited to meet the needs of each individual served in a manner which is safe, effective, efficient and meets recognized standards for quality care.

Case Management

Activities are carried out in collaboration with individual served, as well as family and natural support system with client consent.

Outreach served and encourages the participation by building trust, engaging the individual served in treatment, services, and resources and maintaining ongoing involvement.

Coordination of assistance with crisis intervention and stabilization services as appropriate.

Assistance with achieving goals and objectives for client’s independence.

Optimizing resources and opportunities through community linkages and social support network.

Assistance with: transportation, housing, budgeting, meal preparation, personal care, and other identified needs.

Linkage with the necessary financial, medical, health care and other community support.

Case management services are provided in locations that meet the needs of the individuals served such as: ALF’s, hospitals, community sites, client’s residence and other shelters.
